3758dbbec3 Bug 1411685: Remove another accessibility related diagnostic crash on Nightly. r=aklotz 
I missed this one in bug 1431404.
This condition is rare and does indicate a problem which breaks accessibility.
However, we aren't getting any closer to diagnosing this as a result of this crash, so it causes user pain without any gain to us.

21e763342a Bug 1403358 Only reference ActivationContext if Acessibility is enabled r=aklotz 
This also enables the crash reporter on the MinGW build, as this is the
only thing blocking that from working.

67a116465c Bug 1298882 - ensure mozilla/mscom/Utils.h is included with uniform casing; r=aklotz 
We install mozilla/mscom/Utils.h, but include it with an all-lowercased
path in some places.  clang-cl warns about this, and even though this is
Windows-only code and will therefore work, it's better to make
everything consistent.
