0ae28f14fb Add CPP_THROW_NEW macro to the build system. This macro should be used when defining a local |operator new| function. It will be set to an empty throw function by default on all platforms except mac CW builds. This will allow us to not crash on systems that expect |operator new| to throw an exception by default and will suppress compiler warnings about how |operator new| should throw an exception instead of returning NULL.

5cbfae0613 Convert the 4 tables of the RuleHash from nsHashtable to pldhash. Encapsulate the differences between tables, and the quirks/strict differences within the id and class tables, in the 6 sets of hashtable ops. Remove mCaseSensitive member and related code from AtomKey and remove nsInt32Key completely. Null-terminate lists of RuleValues rather than using a special mEndValue, and build the lists in the reverse order (reversing the meaning of their index). 3047ee4de6 Remove bitrotted DEBUG_RULES code. Clean up RULE_HASH_STATS debugging code. Add comments to HasStateDependentStyle, and speed it up by making IsStateSelector match only what it needs to. 4b63835325 Fix up and enable hierarchical :hover and remove the hacks that made :hover work for anchors without it. Pass mask rather than pseudo-class to ContentStatesChanged notifications. 741a1857ba Second try at fixing bug 130778. Create a new content interface, nsIMenuElement, which is implemeneted on nsHTMLSelectElement, nsHTMLOptGroupElement, and the newly-created nsXULMenuElement. Create a nsXULMenuElement instance for XUL menubar and menupopup tags. Use this interface to set and get the active item from the XUL menuframe code and the :-moz-menuactive pseudoclass matching code. 367cc93c8f - Implement the :checked CSS pseudoclass which maps to the "selected" property on option elements.
- Eliminate the _moz-option-selected attribute; move the actual selected state into the option content node.
- Change all users of _moz-option-selected to use :checked.
- Add a third parameter to nsIDocument[Observer]::ContentStatesChanged to indicate which pseudoclass changed, this is used for optimizing handling of :checked state changes.
