After the fixes in this patch, the REQUIRES_UNIFIED_BUILD declaration is no
longer needed, so I'm removing it here. This directory now builds successfully
with that removed and with ac_add_options --disable-unified-build in the
mozconfig.

Converts layout.css.ruby.intercharacter.enabled to a static pref and removes the associated function nsLayoutUtils::IsInterCharacterRubyEnabled(). Also removes the macro INTERCHARACTER_RUBY_ENABLED_PREF_NAME, since it was only being used to add the varcache pref.

This simplifies dealing with frames that are pushed/pulled between
continuations during reflow, allows us to avoid the complexity of the
fix to 1459937, and hopefully fixes some of the regressions from bug
1308876.
This disables the changes from bug 1459937 by commenting out a single
line in ReparentFrameInternal in nsBlockFrame.cpp, but all the added
code will be removed in the following patch.

Additionally, this patch makes `nsFrame.h` stop including `nsIPresShell.h`
and makes each users include `mozilla/PresShell.h` instead. So, this improves
rebuild performance of `nsIPresShell.h` (and `mozilla/PresShell.h` in the
future).
Note that due to `nsIFrame::PresShell()`, `mozilla::` prefix is necessary for
`PresShell` in a lot of classes which are derived from `nsIFrame` even in
`.cpp` files.

frames have probably not been marked dirty (as ReflowInput hasn't dealt with
them when it was constructed), so we need to mark them dirty for proper reflow.
If we don't do that, and they don't fit in the current column, the next column
will only mark its current children dirty, so when pulling back its first
floats from the previous column they will not be reflowed as needed.

We have a fair number of files that have a particular stale version of the MPL
boilerplate. (It was probably originally correct, and then the official
boilerplate changed, and the stale MPL boilerplate continued to propagate via
copypasting from neighboring files into newly-added files.)

nsReflowStatus::IsEmpty() assertions are added after DISPLAY_REFLOW in the
beginning of the Reflow().
A few Reflow() implementations have Reset() calls at the end which are left
in place by this patch (with an explanatory comment added to each). These
ending Reset()s are only needed for cases where a non-splittable frame
passes its own nsReflowStatus to a child's reflow method. Just in case the
child leaves a "not fully complete" value in the nsReflowStatus, the
non-splittable parent frame must clear out the nsReflowStatus before
returning, so that its own parent doesn't then try to split it.

This is the primary patch in this bug, and makes the performance
improvement that fixes this bug.
The assertion count increase for layout/generic/crashtests/1015844.html
is accompanied by a layout change in the testcase as well. However, I'm
not planning to fix it in this sequence; fundamentally columnsets with
specified heights inside a paginated context (like another columnset) do
not work in any reasonable way, and changing the number of times we
reflow them can change the layout. At least, assuming I didn't lose
something in the process of simplifying the testcase.
ISSUES:
- may make block inside XUL worse in performance by marking dirty more (see subdoc in Firefox UI, or text control innards?)
