- combine nsICharsetConverterManager2 and nsICharsetConverterManager
- get rid of nsIAtom in most of the methods
- provide versions of getUnicodeDecoder/Encoder which don't do alias resolution
- change all charset types to ASCII strings
- clean up some other i18n APIs which could be simplified
- fix all consumers of all changed i18n interfaces

Remove nsIStyleContext (bug 114713), r/sr=dbaron. Changed lots of methods to use nsStyleContext* or already_AddRefed<nsStyleContext> as return values, depending on whether a reference is returned. Turn on nsRefPtr (bug 104346), and change nsDerivedSafe to not declare AddRef and Release explicitly virtual, r=jag, sr=dbaron.

Bug 77834: Fix problem where form control state is being restored to the wrong form control or not restored at all when going back and forth through session history. The problem was that the hash key we used was the content ID, which was not consistent from page load to page load. This checkin enables mozilla to support the printing of selection, the printing of page ranges, and

the printing of headers and footers.
Printing of selection is implemented by the frames figuring out if they are in the selection and painting
if they or not they they don't paint. This also only allows the printing of the first page of
selections, alothough it is well documented where this is implemeted so it can be removed.
