The ghost window checker examines windows in all sorts of weird states,
apparently including when the docshell on the outer is null. Add some
fallible variants of the tab group methods on inner and outer windows
so it can call one of those instead.

I missed in bug 1487216 that the pres arena memory reporting assumes that the
entry indices are frame class ids, which means that we're reporting some display
list arena entries as frames, which is obviously wrong.
Cleanup a bit nsPresArena to remove the custom id concept, and report also
individual display item type memory usage.

For now I added everything to the same bucket, but I wrote this so it should be
easy to add more buckets as needed (either to mArenaSizes, or more specific ones
like the style system has). But this is probably enough for now.

These are owned by the element and not referenced from the stylesheets.
They're referenced from the rule tree, but the rule nodes don't measure their
style source (since they're non-owning).
So unconditionally reporting them even though it's a refcounted object is ok.
While at it, remove some other fields from the old style system that are no
longer used.

Summary: Really sorry for the size of the patch. It's mostly automatic
s/nsIDocument/Document/ but I had to fix up in a bunch of places manually to
add the right namespacing and such.
Overall it's not a very interesting patch I think.
nsDocument.cpp turns into Document.cpp, nsIDocument.h into Document.h and
nsIDocumentInlines.h into DocumentInlines.h.
I also changed a bunch of nsCOMPtr usage to RefPtr, but not all of it.
While fixing up some of the bits I also removed some unneeded OwnerDoc() null
checks and such, but I didn't do anything riskier than that.
This is on top of bug 1486623.
While at it removed the 'stale computed styles' report, since it was unused
since I removed the undisplayed maps.

Adding the Places* files into unified sources pushed the
unified sources into a situation that exposed a strangely
large number of errors. This seems to be the minimum set of
changes I could make to resolve all of the issues.

Rather than recording how many leaked (ghost) windows we see at
various times, I think it will be more useful over all to record what
the maximum number of ghost windows we see during a single
ping. Hopefully this will show up in a way that generates automated
alerts better than GHOST_WINDOWS.
If this works out, I'll make this telemetry measure permanent and
remove GHOST_WINDOWS.

nsStyleStructList.h was initially made generated in bug 873368 to avoid
manually maintaining boilerplate for if-dispatch, while the if-dispatch
was replaced by jump table in bug 1171842, so the boilerplate went away.
However, in bug 1122781 (before bug 1171842), boilerplate for dependency
check, so it still needs to be generated.
The dependency table is removed in the previous patch, so we no longer
have any boilerplate in the style struct list, and thus it doesn't need
to be generated anymore.
