Bug 1400459 devirtualized nsIAtom so that it is no longer a subclass of
nsISupports. This means that nsAtom is now a better name for it than nsIAtom.

We make the NodeInfoInner members raw pointers, and AddRef/Release
them manally in the constructor/destructor of NodeInfo, and in the
cases where NodeInfoInner objects are created for hashtable lookups,
no further refcounting is performed.
If the mathml.disabled preference is true, treat <math> and other MathML
elements as generic XML elements.
This patch disables the rendering code of MathML however preserves the
namespace so to reduce the breakage.

The bulk of this commit was generated with a script, executed at the top
level of a typical source code checkout. The only non-machine-generated
part was modifying MFBT's moz.build to reflect the new naming.
CLOSED TREE makes big refactorings like this a piece of cake.
