640685cbe9 Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. ba58162e96 Remove nsIStyleContext (bug 114713), r/sr=dbaron. Changed lots of methods to use nsStyleContext* or already_AddRefed<nsStyleContext> as return values, depending on whether a reference is returned. edc54e6926 Replace the BodyFixupRule with code in nsCSSRendering to fix all the problems with dynamic style changes and backgrounds on the root element and the BODY element. a1ed5b3a4f This checkin enables mozilla to support the printing of selection, the printing of page ranges, and 
the printing of headers and footers.
Printing of selection is implemented by the frames figuring out if they are in the selection and painting
if they or not they they don't paint. This also only allows the printing of the first page of
selections, alothough it is well documented where this is implemeted so it can be removed.
