662d805035 More nsIPresContext deCOMtamination (bug 229371). Removed GetBaseURL, rename GetMedium to Medium and inline it, remove virtual style resolution methods that forward to style set (callers will call style set methods directly). 640685cbe9 Change all callers of nsIPresContext::GetShell() to use the non-refcounting versions. Most callers now use PresShell(), a few that were checking for null conditions are using GetPresShell() -- this will be remedied once we can ensure that a prescontext never has a null pres shell. 88358c59ac Create a mozAutoDocUpdate class to handle automatically calling 
BeginUpdate/EndUpdate on documents.  Use this in a bunch of places to make sure
that EndUpdate is called in the face of early returns.  Bug 131775, also fixes

c4f180a7f9 Make sure that Init() failures for image frames don't mean no alt text. Also 
fixes a crasher when reframing fixed-pos frames.

This change adds an assertion in ConstructHTMLFrame that InitAndRestoreFrame
should succeed (unless it returns NS_ERROR_FRAME_REPLACED).  This is an interim
change; eventually we want to bail out on errors, but there may be frame impls
that return errors in non-error conditions from Init() (eg nsFrameSetFrame did
that) and we're a little close to freeze.

Changed lots of methods to use nsStyleContext* or already_AddRefed<nsStyleContext> as return values, depending on whether a reference is returned. d1856f6f91 Remove |aForceUnique| parameter from style context resolving methods, and corresponding flag from nsStyleContext, since the only caller was ReResolveStyleContext (on the assumption that other callers used it).
